The fees quoted include the $250.00 non-refundable registration fee, a program of instruction, supervision, room and board for the designated period, camp approved expenses for camp sponsored day trips and activities, craft materials, laundry services, insurance, gratuities, and transportation with baggage from Philadelphia, New York, Massachusetts and New Jersey to camp.
The undersigned acknowledges that he/she is the parent or guardian of the camper designated above and has full authority to enroll the child in a summer camp program. The undersigned further acknowledges that the information contained in this application is true and accurate. The underigned, by supplying the credit card information below and signing this agreement gives the camp permission to apply tuition charges to the card listed below.
Each applicant must provide French Woods with a deposit in the amount of $250.00 at the time the original application is submitted for processing. It is understood that $250.00 of the initial deposit shall be a non-refundable registration fee. Any other payments that have been made on the account, shall be refunded only if the applicant provides French Woods with a written request for such a refund prior to June 1st, 2012. Thereafter, no refunds will be given for any reason. All refunds are made on or about July 1st. The balance of tuition quoted herein shall be due and payable by June 1, 2012 and interest in the amount of 1% per month will be added to the balance. In the event that the tuition fee is not paid in full by the date the session is scheduled to begin, then all monies on account with French Woods shall be forfeited as and for liquidated damages and the camper’s reserved place in the session shall be forfeited.
No allowance or reduction in the tuition will be given for a camper’s late arrival or early withdrawal. No assignment of this contract by either party shall be valid or binding on the other unless the assignment is in writing and approved by written consent of the other party.
Authority is hereby granted without limitation to the camp and its assigns to make decisions and/or authorize treatment of the above named camper with regard to all medical, surgical or dental matters that may arise while the camper is in the care of French Woods including, but not limited to, all matters regarding hospitalization, surgery, injections, medication, and/or anesthesia. If it is necessary to obtain off camp medical or surgical or dental services for the camper, such expenses shall be paid by the parent and/or guardian. The parent(s) and/or guardian(s) are responsible for any out of camp medical, surgical, hospital and/or allergy expenses incurred as result of pre-existing medical conditions. Parent(s) and/or guardian(s) are also responsible for providing adequate quantities of necessary medications and allergy serums to the camp in a pharmacy container with doctor’s instructions at the time the session is scheduled to begin.
The camp is not responsible for damage or loss of musical instruments, clothing, personal effects or personal equipment used during the camper’s stay. The camp specifically advises campers not to bring jewelry or other valuables to camp.
During the camp season, the camper and his/her parent(s) and/or guardian(s) agree to abide by the camp rules and regulations governing health, safety and welfare of the campers and the camp community. Specifically, (a) the possession or use of tobacco, narcotics, liquor or other intoxicants or non-prescription drugs is expressly prohibited both on and off the camp premises; (b) campers may not leave the camp without express permission from the camp director. Violations of these rules or other reasonable regulations will result in dismissal from the camp without tuition refund. The camp reserves the right to dismiss any camper whose conduct is unsatisfactory or inimical to the camp’s and the camp community’s best interests without a tuition refund.
The camp program may include public performances and permission is hereby given for the camper to take part in such performances on and off camp grounds without compensation. The camp may use photographs, statements, articles, names, music, art, films, video tapes taken of the camper or made by the camper in promoting the camp and camp-related activities through advertising, publication and/or exhibition.
This contract shall become binding when signed by an authorized agent/representative of French Woods Performing Arts Camp, Inc. without delivery of a copy thereof to the undersigned applicant. If French Woods is unable to perform any of its obligations under this contract by reason of fire, strike or damage by the elements, or of any unavoidable casualty, the obligations on its part will terminate at once.This contract contains all the terms and conditions agreed on by the parties, and no other agreement, oral or otherwise, regarding to the subject matter of this contract, shall be deemed to exist or to bind any of the parties.

This agreement shall be deemed to have been made in Delaware County, New York, regardless of the order in which the signatures have been affixed and shall be interpreted and the rights and liabilities of the parties determined in accordance with the laws of the State of New York. All matters sounding in contract or in fact relating to the validity, construction, interpretation and enforcement of the contract will be determined in Delaware County, New York. If any provision of this agreement is deemed to be illegal, unconstitutional or otherwise invalid, the remainder of the agreement shall not be affected but it shall be construed as if not containing the particular provision held to be invalid.
It is understood, and the undersigned parent/guardian hereby agrees, that advertising by or on behalf of French Woods Performing Arts Camp, Inc. at locations outside of the state of New York shall not provide a basis for personal jurisdiction over French Woods Performing Arts Camp, Inc. in any court other than those located in Delaware County, New York. It is further understood, and the undersigned parent/guardian hereby agrees, that the pickup and/or drop-off of camper’s at locations outside of the state of New York shall not provide a basis for personal jurisdiction over French Woods Performing Arts Camp, Inc. in any court other than one located in Delaware County, New York.
All disputes and matters whatsoever arising under, in connection with or incident to this agreement or the camper’s stay at French Woods Performing Arts Camp, inc., including travel to and from the camp, shall be litigated, if at all, before the courts located in Delaware County, New York, to the exclusion of the courts of any other county, state or country. The undersigned parent/guardian hereby agrees that the courts located in Delaware County, New York have jurisdiction over the person of said parent/guardian. The undersigned parent/guardian hereby waives any objection that venue in the State of New York is inconvenient or improper.
